Probably for that I'd go with "It had been 5 years since I last saw him," or "It had been 5 years since I saw him last," or something similar. Strictly speaking "It had been 5 years since I had last seen him" might not be incorrect but-awkward. In 1-4, I understand what you are saying in all but would never write 4. And am pretty sure you'd get a red mark on your paper for that one-for style if nothing else. 1 and 3 would be rarer in AE, but I have a feeling that they might not be in BE. I think both would be grammatically correct although 3 a bit unusual and we would tend not to use the present tense in the first verb. Most North Americans would choose "It has been 5 years since I last saw him," or "It's going on 5 years since he worked here," "It's been 5 years since I've seen her." Am always curious to hear what the Brits have to say!
